We are a program of Los Angeles Jewish Health, renowned for providing quality care to vulnerable seniors.The responsibilities of the Nurse Practitioner include, but are not limited to:Responsibilities:Participates in the health assessment and examination of BCSC PACE applicants and participants.Serves as a member of the PACE Interdisciplinary TeamEvaluates the needs and participates in the development of a comprehensive program and nursing treatment for each participant in the BCSC PACE Clinic.Manages minor illnesses, injuries, and the stable, chronic illnesses of participants, according to standard procedures.Evaluates the acute illnesses of the participants.Coordinates appropriate referrals to health care providers in consultation with BCSC PACE physician.Makes nursing home/or home visits when appropriate and as directed by the BCSC PACE physicians.Provides clinical assistance to BCSC PACE's staff registered nurses.Participates in all programs of health restoration, prevention, promotion and maintenance of health.Attends and participates in staff meetings.Duties may include participation in the Inter-Disciplinary Team (IDT) meetings as a proxy and fulfill responsibilities as assigned. Fulfills all administrative requirements, including statistical records, chart entries, etc.Meets with department directors on a regularly scheduled basis, and conduct/participate in in-service classes and supervisory level training programs.Attends and participates in workshops, seminars, etc., to keep abreast of current changes in the long-term care field, as well as to maintain a professional status.Attends and participates in annual in-service training programs for hazard communication, TB management, and blood borne pathogens standard. Actively participants in the Quality Assessment and Performance Improvement (QAPI) process. Consistently meets or exceeds BCSC quality assessment and performance improvement targets.Continually seeks better ways for delivering services and communicating with participants while demonstrating proficiency in delivering age-specific care. Demonstrates respect for and promotes participant rights, including dignity, self-determination, access to care, confidentiality and independence.Understands the importance of community involvement and participates as appropriate in activities that link BCSC to its communities.Consistently meets or exceeds BCSC targets for productivity and customer service targets.Ensure Nursing personnel, on your tour of duty, participate in all fire safety and disaster preparedness drills in a safe and professional manner.Experience and Skills:Certificates & Licenses: Licensed RN & NP, State of California; meets standards of Board of Registered Nursing and the appropriate Business and Professional Code, State of California; successful completion of a program of study, which conforms to Board of Registered Nursing standards; verification of completion of a CPR (BLS) and first aid training. TB screening and successful completion of a health examination by a health care provider is required for employment. Experience: One year of nursing experience at the RN level and some supervisory experience preferred. At least one year of experience working with a frail elderly population.Skills & Aptitudes: Effective communication skills, both written and verbal, are required. Inpatient Social Worker - Geriatric Psychiatry Unit

Join our team and experience a workplace culture that fosters teamwork, growth, and support!The responsibilities of the Social Worker include, but are not limited to: Responsibilities:Assessment / Reassessment: Assists in the completion of an initial psycho-social assessment within 72 hours of a patient admission utilizing input from the patient and patient’s authorized representative. Documents individual and group interventions daily in the attendance record. Completes progress notes including psycho-social evaluation weekly or more often, as needed.Documentation: Writes progress note on admission, weekly, upon discharge and as needed with new concerns, significant change of behaviors, response to interventions and when new interventions are needed as well as discharge plans. Completes daily documentation of individual and group interventions as directed.Discharge Planning / Utilization Review: Initiates discharge planning on admission and coordinates the discharge process with the patient, patient’s authorized representatives and other resources.Treatment planning: Assures that treatment plans include appropriate social service interventions within 72 hours of patient’s admission and are updated with each change of condition, change in behaviors and at least weekly. Assures that the patient / patient’s authorized representatives are involved in development of the treatment plan and sign the treatment plan to acknowledge agreement with the treatment plan.Provides discharge planning and documents medical necessity.Assists patient and patient’s authorized representative select an appropriate after care placement when needed.Assist with therapeutic activities, specifically leading 1-2 patient groups per week.Manages unit case load in an efficient manner and coordinates responsibilities with L.C.S.W.Participates in development, review, revision and implementation of policies and procedures for the Acute Psychiatric Unit as needed.Conducts regular unit rounds with LCSW and other interdisciplinary team members as appropriate to identify care and environmental issues and implement corrections as appropriate.Quality Assurance and Performance Improvement: Actively participates in the Acute Psychiatric Unit Quality Assurance and Performance Improvement (QAPI) program. Reviews indicators for social service on a pre-assigned basis, makes any necessary corrections and makes recommendations for improvement to the QA and PI Committee at least quarterly.Assists with coordinating transfers to other acute psychiatric facilities providing involuntary care when needed.Assures security of environment to prevent exit of patients attempting to elope. Join our team and experience a workplace culture that fosters teamwork, growth, and support!The responsibilities of the Occupational Therapist include, but are not limited to: Responsibilities:Under a physician’s orders, screen and evaluate residents/patients, developing an appropriate care plan.Assist residents/patients with activities of daily living, including dressing, cooking, and eating.Instruct residents, families, and caregivers in the use of adaptive equipment, such as wheelchairs, splints, and aids for eating and dressing.Maintain patient records accurately and in a timely manner.Develop and implement special tools and treatments tailored to the specific needs of patients.Complete required forms and documentation in accordance with company policy and state and/or federal regulations.Experience and Skills:Certified or registered by the National Certification Board for Occupational Therapy (NCBOT).Licensed in Occupational Therapy, if required by state law.One (1) year of clinical experience preferred.Supervisory Responsibilities:May supervise COTAs, Rehab Aides, or provisional licensed occupational therapists according to state practice guidelines.Physical Demands and Environment:Shift work may be required.Employee is frequently required to stand, walk, use hands or fingers, reach with hands and arms, feel, talk, and hear.Employee may occasionally assist in lifting and/or transferring patients weighing between 100 to 250 pounds.Employee may be occasionally exposed to blood or other body fluids, fumes, airborne particles, and toxic or caustic chemicals.In compliance with applicable law, reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position.Join Los Angeles Jewish Health and make a meaningful impact in the lives of seniors in our community.
