The responsibilities of the Social Worker - MSW include, but are not limited to:
Responsibilities:
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
- Must have advanced training in hospital or long-term administration.
- Must be knowledgeable of reimbursement regulations and nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to long-term care administration.
- Must be familiar with the laws, regulations, and guidelines governing personnel administration.
- Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents, staff, and visitors based on whatever maturity level at which they are currently functioning.
- Must be able to communicate policies, procedures, regulations, reports, etc., to personnel, residents, family members, visitors, and government agencies/personnel.
- Knowledge of charting via Minimum Data Set (MDS).
- Experience completing psychosocial evaluations, social histories, cognitive, mood and behavioral assessments.
- Quarterly Care Plan meeting attendance is required and completion of treatment plans within prescribed time frames.
- Experience providing services to the elderly population and their families.
- Familiarity of Resident Rights and Advance Directives.
- Knowledge of Discharge Planning policy and procedures.
- Will act as liaison and referral with psychiatric and psychological service providers as well as ancillary service providers.
- Must be a team player with strong clinical skills.
Experience and Skills:
- Master’s Degree, MSW required.
- Must possess a working knowledge of long-term care operational standards set forth in the Federal Register, Requirements of participation. Experience working with the elderly.
