The Social Service Case Aide’s responsibilities are but not limited to:
Responsibilities:
- Assist the Social Services Department under the direction and supervision of the Team Leader of Social Services or Social Worker in accordance with current guidelines and regulations; and established policies and procedures.
- The focus is to assist each resident and family in adjusting to placement, illness, and plan of care to attain the highest level of functioning.
- Completing inventories of clothing, personal items and labeling.
- Addressing patient issues and concerns by completing investigations.
- Processing letters inviting family members to Care Plan meetings.
- Delivering mail and packages to patients.
- Preparing rooms for new admissions, and
- Temporarily stores items for residents who expire.
- Other duties assigned by the Team Leader and/or Social Worker.
Experience and Skills:
- High school diploma or equivalent.
- Preferred experience with the geriatric population and Dementia residents.
- Preferred completion of the Social Service Designee 36-Hour Certification Program.
- Positive Attitude, team player, able to multitask, and strong Resident Advocate.
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors and government agencies.
- Must function independently and have flexibility, personal integrity, and ability to work effectively with residents, personnel, and support agencies.
- Excellent verbal and written communication skills, cooperation, dependability and flexibility.
- Ability to present ideas and thoughts in a direct, professional manner, both orally and in writing.
- Understanding of Medicare, Medicaid, and Resident Rights.
- Experience in the Assisted Living Waiver Program (ALWP) is preferred.
