Hospice care recognizes the needs of patients who choose to remain where they live, outside of the hospital so they can be surrounded by loved ones when hospitalization is no longer expected to cure their illness. Skirball Hospice provides the support that allows that patient to spend the last stages of life in a loving environment, comforted by friends and family, and free from discomfort.

The responsibilities of the Social Worker include, but are not limited to: 

Responsibilities:

  • Perform psychosocial assessments for patients to enhance existing services and contribute to the treatment plan.
  • Report any changes in the patient's condition or needs to the RN Case Manager or Director of Patient Care Services.
  • Observe and identify safety hazards in the patient’s and organization’s environments, taking appropriate steps to mitigate or eliminate them.
  • Assist the physician and other healthcare team members in understanding the social and emotional factors related to the patient’s terminal illness.
  • Assess suicide risk, mental capacity, and potential child, elder, or dependent adult abuse, taking appropriate action when necessary.
  • Assist with completing advance directives, mortuary arrangements, funeral planning, or facility placement as needed.
  • Provide assistance to staff, patients, and families in identifying third-party financial resources and planning for coverage of services.
  • Offer information and referrals to community resources and legal entities.
  • Advise and counsel hospice staff, patients, families, or community agencies in managing emergency situations or crises.
  • Provide consultation and education to the community to raise awareness of hospice services, acting as an informational and educational resource for the community, agency personnel, and volunteers as appropriate.
  • Maintain professional development through academic studies, seminars, and workshops.
  • Attend and participate in in-services, staff meetings, and case conferences.
  • Complete all documentation accurately and legibly.
  • Adhere to hospice policies, procedures, laws, regulations, and standards of practice.

Experience and Skills:

  • Master’s degree in social work from a school accredited by the Council on Social Work Education.
  • L.C.S.W. preferred but not required.
  • Clinical experience relevant to the psychosocial needs of patients and families.
  • Knowledge of psychosocial theories and experience with psychosocial treatment techniques for adults, children, and groups.
  • Ability to work with terminally ill patients and their families.
  • Excellent oral and written communication skills.
  • Valid driver’s license and auto insurance.
