Skirball Hospice & Palliative Care is an established hospice agency located in the San Fernando Valley that provides hospice & palliative care services throughout Los Angeles County. We are seeking a professional hospice education and quality coordinator to join our team. This role is responsible for the ongoing participation in staff education, quality focused review of ongoing operations, documentation reviews and routine feedback to responsible staff, assisting the agency in QAPI activities, participation in implementation and development of policies and procedures to comply with all regulations, participation in performance improvement plans, and other duties necessary to provide high quality of care to patients.

Job Functions/Responsibilities:

  • Assists in compliance with local, State and Federal laws and regulations.
  • Evaluate, prioritize, develop and implement staff education on an ongoing basis through ongoing review of medical record documentation.
  • Collaborates with hospice team members, including Executive Director, Director of Patient Care Services, RN Case Managers and Physicians, to coordinate education and quality assurance activities.
  • Serves as a resource and support system to staff in problem solving and determining appropriate interventions.
  • Assists in oversight of team members to ensure compliance and quality are maintained. 
  • Assists in oversight of performance improvement plans developed through the QAPI process and identified needs.
  • Oversees that clinical documentation is completed in an accurate, timely and legible manner.
  • Participates in the agency’s QAPI program and/or in response to concerns related to the patient/patient care.
  • Perform patient visits to observe, evaluate, and participate in care as needed.
  • Assist the Director of Patient Care Services in program planning and development.
  • Conducts orientation, competency evaluations, 90 day and annual review of all team members.
  • Proactively conducts root cause analysis of identified opportunities and take corrective action
  • Contributes to the development and delivery of in-services, staff meetings and case conferences.
  • Reads, interprets and enforces hospice policies and procedures.
  • Acts as a positive and collaborative agency team member to achieve goals, targets and priorities.

Education and Experience:

  • Must have current LVN license by the State of California.
  • Two years or more experience in hospice required.
  • Valid California driver's license with good driving record.
  • Experience with education and/or quality assurance activities for hospice. 
  • Robust knowledge and understanding of the clinical delivery of hospice care services. 
  • Strong knowledge of hospice eligibility requirements as well as federal, state, local and regulations as applicable to hospice.
  • Exceptional oral and written communication and presentation skills, with the proven ability to effectively communicate with patients and caregivers, hospice team members, business associates, and prospective referral sources. 
  • Comprehensive working knowledge of various computer software systems and EMRs.
  • Comprehensive working knowledge of the current healthcare environment.
  • Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
