The responsibilities of the Clinical Admissions Nurse (LVN/RN) include, but are not limited to:
Responsibilities:
- Review incoming clinical information for new intake residents from home, other long term care facilities, and hospitals as well as LAJH residents being re-admitted from the hospitals, and obtain necessary information to make an informed decision regarding appropriate placement based on resident characteristics and clinical needs.
- Provide recommendation for admissions to DONs/Administrators or rationale for denial to Director of Admissions.
- Verify insurance eligibility for Part A for SNF and RCFE residents’ admission and/or readmissions.
- Obtain prior authorization for skilled services for appropriate level of care from managed care health plans prior to admissions and/or readmissions.
- Establish cooperative and trusting relationships with community hospitals, physicians, health organizations and care managers.
- Provide timely communication if resident is approved or denied admissions to referring hospital, individual or community facility.
Experience and Skills:
- RN license preferred, minimum LVN license.
- Background clearance, TB screening, and successful completion of a health examination by a Los Angeles Jewish Health Nurse Practitioner or M.D.
- Strong clinical and documentation skills.
- Excellent communication and supervisory skills, with the ability to work effectively as part of a team.
