The responsibilities of the Care Coordination Specialist include, but are not limited to:
Responsibilities:
- Coordinate and manage patient care across multiple healthcare providers, health plans and managed healthcare organizations, ensuring timely follow-ups and continuity of care.
- Assist residents in navigating a comprehensive healthcare delivery system under LAJH.
- Verify insurance eligibility, obtain prior authorizations, and facilitate referrals to specialists or external healthcare services.
- Review patient records, track progress, and ensure adherence to treatment plans.
- Communicate effectively with patients, families, healthcare teams, and insurance providers to ensure efficient care delivery.
- Assist in discharge planning and transitions of care, ensuring patients receive necessary post-care services.
- Maintain accurate and up-to-date documentation in electronic health records (EHR) and/or other software systems.
- Identify gaps in care and collaborate with multi-disciplinary teams to improve patient outcomes.
- Track patient progress, maintain accurate documentation, and ensure compliance with healthcare regulations.
Experience and Skills:
- Clinical background (LVN) and experience in healthcare coordination, case management, or patient navigation.
- 2+ years of experience in care coordination, case management, patient advocacy, or healthcare administration is preferred.
- Strong understanding of medical terminology, CPT codes, ICD-10 codes, and prior authorization processes.
- Excellent communication, problem-solving, and organizational skills.
- Proficiency in electronic health records (EHR).
- Prior experience in insurance verification, prior authorizations, and utilization review is highly desirable.
- Experience in rehabilitation, managed care, or skilled nursing settings is a strong advantage.
- Familiarity with managed care plans and programs including Medicare Advantage, managed care Medi-Cal, Dual Eligible Special Needs Plans (D-SNPs) and HMO/PPO plans.
- In-depth knowledge of HIPAA regulations and patient privacy laws.
- Understanding of post-acute care services, rehabilitation processes, and discharge planning.
- Passion for patient advocacy, improving healthcare access, and financial steward of utilization and resources.
Education & Certifications:
- Licensed Vocational Nurse (LVN).
- Certified Case Manager (CCM) or Certified Professional in Healthcare Quality (CPHQ) is a plus.
- Additional certifications in care coordination, medical billing, or patient navigation are beneficial.
- Bilingual proficiency (e.g., Spanish) is a plus, especially in diverse patient populations.
